Patient had twin pregnancy, PROM at 19 weeks and ended up delivering at 19.3 weeks. One was born alive and lived a couple of hours and one was stillborn. I know you don't typically charge a delivery code under 20 weeks, but what about since one was born alive? Dose that make a difference?

No it does not. You bill the event using inpatient E/M codes - admission, subsequent care, discharge day management and any documented prolonged E/M services.
