Melanie ,
Please advise ... Our physician treated a patient for all of her prenatal and postnatal care including delivery of baby boy . The baby boy is then brought into our OB/GYN office and the Physician performs a circ ( 54150) . Can our physician bill for a 99204 and 54150?

Probably not. The E/M can only be billed if it was separate and significant from the circumcision procedure which you have said was the reason for the visit. 54150 is all you should bill. It is also highly unlikely that your physician can bill for a newborn visit (a 99204 would be way too high a level of service for a newborn in any case unless he had a disease) and an ob/gyn will not usually be reimbursed for routine child care (99462). Remember that all procedures include a quick exam of the patient prior to doing the procedure, explaining the procedure to the patient or guardian, applying the anesthetic, surgical prep, doing the procedure, checking for bleeding, documenting the procedure, talking to the patient or guardian following the procedure and instructions.
