Thank you so much for your opinion.

CPT for control of bleeding? I am not sure about this as Dr Y located and put on hemoclips.

51860 for bladder repair. I am thinking a simple repair, not complicated. The physician I am billing for had nothing to do with her previous surgeries, but was called in.

PREOPERATIVE DIAGNOSIS:
Cesarean section
Hysterectomy
Bladder laceration
Bleeding

POSTOPERATIVE DIAGNOSIS:
Same

OPERATION:
1. Cystotomy closure
2. Control of hemorrage

SURGEON:
DR Y


ASSISTANT:
Dr X


ANESTHESIA:
General

ESTIMATED BLOOD LOSS:
300 mL

INDICATIONS:
35 yo with a placenta accreta and bleeding

FINDINGS:
4 cm cystotomy. Bleeding from cuff and right broad ligament

SPECIMEN:
None

PROCEDURE:

Called into case with bleeding and cystotomy. Please see other report for hysterectomy and cesarean op note.

The bookwalter retractor was then placed and attention was turned to the pelvis. The bowel was then packed back and the the vaginal cuff was noted and closed with 0- PDS x 4. Good closure noted. The bleeding from the broad ligament was controlled with hemoclips. The bladder cystotomy was closed with chromic and PDS in two layer closure. Good closure noted as back fill with methylene blue revealed no holes. Irrigation of the pelvis noted to be hemostatic. floseal and fibrillar noted hemostasis in the pelvis. A JP drain placed in pelvis and out in the LLQ.

The case was returned back to Dr. X for closure.
