Normally you would follow the rule that always applies when a procedure is converted to open, but in this case, the 2 open codes that represent this procedure (interstitial ectopic) do not match what was done. The closest code is 59136-52 as he did not remove any part of the uterus, but usually the open code would take a modifier -22 to account for the laparoscopic part as additional work. In this case I would advise 59136-52 and 49320-59 as they are bundled otherwise.
