When my doctor performs a sonohysterogram in the office we bill 76831 and 58340. If the sonohyst cannot be completed due to a stenotic cervix or other issues which code do I put the -52 modifier on to show that it was not completed? The catheter and saline are still being used........ Thank You!!

In that case I would bill as you suggested: 58340-52 and 76831.

Here is one note from a patient: Transvaginal ultrasound was performed prior to the injection of fluid. A speculum was inserted into the vagina to visualize the cervix. The cervix was cleansed with Betadine. A balloon-tipped sonohysterogram catheter was inserted into the uterine cavity using aspetic technique. Sterile normal saline was injected slowly under real-time sonographic imaging using a total of 15 ml.
Unable to distend the endometrial cavity.

If the cervix cannot be entered due to stenosis, how are you using the catheter and saline? I would need to see a description of the work actually performed to assist you with this.
