When should you use the 90384 and when should you use the J2791. If it is payer preference is ok to bill just the J2791 as long as you are being reimbursed?

It is definitely payer preference driven. But in all cases, the payer is still going to require that you include the NDC number of the product dispensed no matter which code you use.
