I called a Vanderbilt Clinic physician to make an appt for a pt for
evaluation of hypothyroidism. We received a fax back to confirm a
"consultation request" quoting new Medicare guidelines concerning
consultation services eff 1/1/06 for documentation by the requesting
physician for intent of referral for consultation. Anybody else =
familiar or
requiring a form like this??

This form is not an official form..but one we created to support the revised changes required for the requesting(outside) provider. (We have an internal process in place for our internal providers within our system)...I am happy to send/share it if you could send me a personal email at kjorgensen@peacehealth.org...

So, in order to make sure that the originating dr. has written documentation
of the request for consult, the form is just our insurance.....

You must have a written form from the referring doc??? So, if a dr office
calls to send a pt and the receiving doc documents in the chart, "pt sent
here by Dr. so and so" , then sends report back, that is not significant?? I
guess I am behind on this.

Yes...we are doing the same for outside providers...we also have an internal process for in house.
