In this case the lysis is not bundled with the other procedures described. I would go with 58660, 49322-51, 49321-51. He did not described removing any endometrial implants which would have been 58662, but rather did a Bx of the cyst wall after draining them. You should also note that 58660 and 58662 are a permanently bundled pair.
