what cpt codes to use for this procedure?

CPT has no open or laparoscopic code that covers untwisting an ovary which is the procedure described here. And of course the cautery was to control bleeding which is included in every procedure. I might go with the unlisted code 58679 and then compare the work to 58825 (with 20.44 RVUs). However 58825 is an open abdominal procedure and is meant to be reported when the ovaries are moved out of the way prior to chemo therapy so this is just a ballpark and you should consult the surgeon with regard to a comparison code. You could also compare the work to 49322 (with 10.85 RVUs) or maybe even 58660 (with 19.65 RVUs). Again, this is a call the surgeon needs to make.
