I have never had this happen before but we had a patient who delivered her baby at home. The Dr. met her in the E/R where he delivered the placenta and performed the 1 degree laceration repair and dictated the delivery note. How do I bill this? Thank You!

You have 2 choices: 59400-52 or itemize the antepartum, episiotomy, placenta delivery, hospital admission and care and then the PP care. I would opt for the 59400-52. You will need to inform the payer what part of the work was not completed. When the global codes were first valued the work was calculated as follows:

Antepartum care - 41% of the work
Admission H&P and labor management - 36% of the work
Vaginal delivery - 15% of the work
Postpartum care (includes inpatient and outpatient visits) - 8% of the work
