Hello Melanie,

When reviewing GYNONC follow up visits notes, what would be the HPI elements for a follow up for Grade 1 endoumentrial cancer. The GYNONC follow up notes repeats the history of present illness on every note
Thank you for your assistance.

Location: endometrium
Severity-Grade 1
Associate signs and symptoms-denies vaginal bleeding

That's all I could get out of this note as well. However, this visit was most likely counseling dominated in which case you only need to have the discussion documented and total face-to-face time with a statement about more than 50% was spent on counseling and then HPI does not come into the equation. He did not do an extensive exam and the reason for the visit was to discuss treatment.
