Implanon/Nexplanon Removal, Insertion, Removal & Insertion

There are several codes which to choose from and I'd like to know the difference as to when they are appropriate to use.
For instance if a patient comes in because the device she had expired and they insert a new one, would that be a removal with reinsertion (11983)? or
a Removal 11982) and Insertion (11981) separately?
Also What is the difference between 11976 (capsules) and 11982 Removal (mon biodegradable drug delivery implant):

11976 Removal of Implantable contraceptive "capsules"
11981 Insertion, non biodegradable drug delivery implant
11982 Removal, non biodegradable drug delivery implant
11983 Removal with Reinsertion, non biodegradable drug delivery implant

OCA did an article on IUDs a few year's ago when the old codes for implantable contraceptive capsules were deleted (see: Reader Question: Correct Your Implanon/Nexplanon Codes on Your Encounter Form - Published on Tue, Jan 17, 2012). 11976 is only for the removal of Norplant, a product that consisted of implantable capsules rather than the rod used with Implanon or Nexplanon. If the patient is in for removal of Norplant you use 11976 and then 11981 for insertion of the a rod system at that same visit. If a rod is being removed and replaced at the same visit you code 11983.
