Hello Melanie,

Would you give 4 HPI for the below example.

35 year old female with secondary (context) infertility (location) for three years (duration) unresponsive to ovulation induction (modifying factor).

No. Location is not a disease or symptom, but refers to where the complaint is generating from (body part). Secondary infertility is the inability to become pregnant or to carry a baby to term after previously giving birth to a baby to I would call this whole description an associated sign or symptom. So by my measure you have 3 HPI elements, not 4.
