CLIA regulations apply to all laboratory tests administered in the US no matter who the payer. In this case if you are performing this in your office lab you must add the QW modifier to get it paid. This tells the payer that you have qualified to perform CLIA waived tests. There are some tests on the CLIA waived test list that do not require the QW modifier, but 85018 always requires it.
