Hi Melanie! Pre op dx was abnormal endometrium on ultrasound.........What cpt code would I use for hysteroscopic resection of adhesions and D&C? The uterus was sounded to 9cm. the hysteroscope was placed into the uterine cavity. There were vertical uterine scarring visualized and hemosiderin stained endometrium on the right fundus. Using the Myosure Lite, the uterine scarring was resected and the hemosiderin stained endometrium were biopsied. No other uterine pathology was able to be visualized. At this point the hysteroscope was removed. Gentle sharp curettage was then performed. All instruments were removed.....

If the path report comes back with a diagnosis of adhesions then you can report 58559. And he also did a biopsy and D&C so you can also report 58558. These two codes are not bundled under CCI rules so you should not have to use a modifier -59. 58559 would be listed as your primary code.
