Hi Melanie,

The physician performed a TAH BSO.

The physician noted that BSO was done at patient's request.

The CPT code included with or with/out BSO

Is there a diagnosis that I should use stating patient's request.

You can use a Z40 code if you wish, but as the CPT code includes removal whether there is an indication or not, you don't have to. Look at the Z40 codes. There is no code for patient choice, only if it was done for prophylactic reasons which really should be documented in the record if you are going with the Z40 code.
